Alteplase is only given intravenously and should be administered as soon as possible after onset of symptoms. In Myocardial infarction, Alteplase can be given as an accelerated infusion or a three-hour infusion. The accelerated infusion is given at the following doses: For patients weighing more than 65 kg, the recommended total dose is 100 mg administered as follows: a 15 mg bolus followed by a 50 mg infusion over the first 30 minutes followed by a 35 mg infusion over the following 60 minutes. For patients weighing less than or equal to 65 kg: the dosage is a 15 mg bolus followed by a 0.75 mg/kg infusion over the first 30 minutes and a 0.5 mg/kg infusion over the following 60 minutes.Three hour infusion is given as follows: For patients weighing more than 65 kg; the recommended total dose is 100 mg administered as follows: a 10 mg bolus; followed by a 50 mg infusion over the first hour followed by a 40 mg infusion over the following two hours. For patients weighing less than or equal to 65 kg, the dose is adjusted so that the total dose does not exceed 15 mg/kg. For Pulmonary embolism a total dose of 100 mg should be administered over two hours. The most commonly used regimen is a 10 mg bolus over one to two minutes followed by a 90 mg infusion over two hours. The total dose should not exceed 15 mg/kg in patients with a bodyweight less than 65 kg. The total dose of Alteplase should not exceed 100mg when it is being used for the treatment of acute myocardial infarction or pulmonary embolism as a dosage greater than 100mg has been associated with an increase in intracranial bleeding. The total dose used for the treatment of acute ischaemic stroke should not exceed 90 mg for similar reasons. The recommended dose for the treatment of acute ischaemic stroke is 0.9 mg/kg bodyweight to a maximum of 90 mg infused over 60 minutes with 10% of the total dose administered as an initial bolus.
