Physeptone - Basic Information

 

Indication of Physeptone:

Methadone is useful in most cases where morphine would not be suitable. The most important indication is severe pain of visceral origin. It is not recommended for use in patients who are able to walk around.

Action of Physeptone:

Methadone is an opioid analgesic similar to morphine. Opioid analgesics work directly on the central nervous system, where they act on specific opioid receptors. By interacting with these receptors Methadone can cause analgesia without causing loss of consciousness, although sedation is commonly seen. Opioid analgesics also act on the respiratory control centre of the brain, causing a decrease in breathing rate and depth. Due to an effect on organs that contain smooth muscle (stomach, intestines, gallbladder etc), opioids also cause a number of gastrointestinal effects. The major difference between Methadone and morphine is that Methadone tends to show persistent effects with repeated administration.

Dose Advice of Physeptone:

- 5-10mg orally or by intravenous or subcutaneous injection- Repeat 6-8 hourly initially, then adjust depending on patient response- Caution should be exercised with repeat dosing in the ill or elderly, as the half-life may be extended in these patients.- Chronically, Physeptone the lowest effective dose should not be administered more than twice daily.- Combination with non-narcotic analgesics may increase effectiveness.

Common side effects of Physeptone:

Respiratory depression (breathing becomes slower and more shallow) is the major side effect of Physeptone.Other commonly reported adverse effects include: - Nausea.- Vomiting.- Dry mouth.- Constipation.- Dizziness.- Drowsiness.- Lightheadedness.- Sweating.- Confusion.- Decreased urinary output.- Local irritation (if administered subcutaneously).

Uncommon side effects of Physeptone:

- Euphoria (at higher doses in tolerant patients).- Low blood pressure.- Collapse.- Generalised oedema.- Development of breast tissue in men (long term use of high doses).- Mild withdrawal symptoms (if tolerance has developed).
